Load Balancing and 2 Factor Authentication for RDS in Windows 2012

A couple of Questions if I may :)

Does anyone know of a way to add Active Card (2 factor auth) to the front end of an RDS Deployment without having to utilise TMG2010 or User Access Gateway? All documentation I have found at present points to using some form of Reverse proxy, this isn't an issue I'm just checking if other options are available. For instance on Citrix you simply enable it on the Web interface Servers.

The Second Query relates to Load Balancing - I am looking to Load Balance the RDWA and the RDCB, the intention is to use Hardware Load Balancing (Cisco ACE). Is the best option to simply have a couple of Internal VIPS - Say RDWA.mydomain.com with 2 hosts behind it that then point to a VIP With 2 RDCB Servers behind it?

For the Database we will be using SQL 2012 Cluster to make this component HA. Really the Q is how to scale the Web and RDCB Servers
